Filling in the form
How do I fill in the form?
We prefer you to claim over the phone because it is faster, but you can also
print a form (1493KB - 17 November 2011 - PDF Help) and fill it in yourself.
The form is quite long and asks for a lot of information so please take time to read all the notes. Please provide all the information and documents we ask for as quickly as possible. If you do not, we will have to contact you and this will delay your claim.
National Insurance Number
You must provide documents to prove your National Insurance Number. If you have a partner, they must prove their National Insurance Number too. We can accept any of the following documents:
- National Insurance card
- payslip
- P45 or P60
- tax letters
- letters from the Department for Work and Pensions
- letters from HM Revenues and Customs
- bank statements
If you can't provide your National Insurance Number, or proof of it, please tell us.
Evidence
You must also provide documents to prove your:
- income
- capital
- earnings
- self-employed earnings
- rent.
We will need to see original documents. We cannot accept photocopies. If you are having problems getting any documents, please tell us.
You can take your form and documents to one of our benefit offices. An experienced member of staff will check your form and copy your documents. You can also post your form and documents to us at:
